Freeze and Cancellation Request Form

Please complete the form to communicate your request. Our annual memberships require a 30-day notice for cancellations which is the earliest effective date to request. An early cancellation fee applies if cancelled during the initial year of membership. Your last invoice will not be prorated if it happens to fall mid-billing cycle. We will confirm the processing of the cancellation typically within 48 hours. You will receive actual confirmation from our billing system on the day of the actual cancellation. Your membership is not formally cancelled until you receive this confirmation. ADDITIONAL FAMILY MEMBERS MUST BE LISTED IF THEY ARE CANCELLING.

Freezes are $10 per month and will start the day of your next billing. We freeze up to three months annually. Paid in full memberships are not eligible for freezes.
